The pigmentation usually fades over three to four months, once the course of chemotherapy is full and the bleomycin is discontinued. It presents with linear streaks of erythematous papules or petechiae, which often evolve to flagellate pigmentation much like that induced by bleomycin18. History and Epidemiology Toxicoderma brought on by shiitake mushroom ingestion was first described within the Japanese literature in 1977. Since then, over one hundred fifty circumstances of "shiitake dermatitis" have been reported, principally from Japan and Europe. Pathogenesis Lentinan, a thermolabile toxin in shiitake mushrooms, might be answerable for this dermatosis. Clinical Features In shiitake mushroom dermatitis, pruritic erythematous papules, vesicles, and edema develop on the face, scalp, trunk, and extremities 1�2 days after mushroom ingestion. Scratching results in lengthy, flagellate streaks composed of erythematous papules and/or petechiae on the extremities and the trunk, with sparing of the difficult-to-reach mid higher again. Diffuse erythema and edema in photoexposed areas have been observed in half of Japanese sufferers with shiitake mushroom dermatitis, and fever and malaise are occasional related findings18. The full extent of skin involvement is often apparent by the point the child is 2�3 years of age22. Occasionally, sufferers with "pigmentary mosaicism" will develop both hypo- and hyperpigmented streaks, which can be referred to as "linear nevoid pigmentation". Pathology Spongiosis and necrotic keratinocytes are seen inside the dermis along with a lymphocytic dermal infiltrate, which is admixed with melanophages in instances with hyperpigmentation18. Treatment the acute dermatitis may be handled with topical corticosteroids and oral antihistamines. Differential prognosis the differential diagnosis contains other problems characterised by hyperpigmentation alongside the lines of Blaschko (see Table sixty seven. Some epidermal nevi initially have minimal or no palpable component, but they eventually develop some elevation. A similar violaceous to brown-colored net-like pattern can be seen on the decrease extremities of sufferers with livedo reticularis. Reticulated hyperpigmentation is an occasional function of other pores and skin problems, corresponding to its occurrence on the anterolateral neck in patients with atopic dermatitis ("atopic dirty neck") and in association with epidermolysis bullosa herpetiformis. When a true reticulated pigmentary disorder exists, the family historical past should be reviewed and the patient carefully examined for associated abnormalities. The following part discusses prurigo pigmentosa and six genodermatoses related to reticulated hyperpigmentation.
